How do you go about repairing bad or failed SEO?

How do you go about repairing bad or failed SEO? Lets say you made some mistakes and have a very bad or low SEO score. Can you easily fix it and bring yourself back up to the top? Or will it be a long long long road for you? What do you all think about this topic?

I would probably try to fix some of the problems, then go under a different domain

You can always repair SEO, if you know what you have to do. look for better keywords, see where you went wrong and start working on it step by step. I used to get the wrong keywords all the time, so I just change my strategy and tried to get better results
